Tick box for key health conditions that may need to be taken into account during classes and for contra indicators that have a bearing on the individual requiring medical consent to take part in classes.

We collect or use the following personal information for health and safety and to identify any medical conditions to be aware of or that are deemed contra indicators for beginning meditation:
Name, address and contact details
Date of birth
Emergency contact details
Health information (including whether individuals have high/low blood pressure, anxiety, hearing/sight/mobility limitations AND whether individuals have epilepsy/schizophrenia/depression/are under supervision with a mental health provider).
The above information is subject to additional protection due to its sensitive nature (health information).

· By collecting some key health data we are both ensuring that classes are made as accessible and inclusive as possible (which is in the interest of the individual) and identifying individuals who have contra indicators whom we ask to declare that they have medical consent before they begin meditation classes. This protects any individuals who should be seeking medical consent before beginning meditation.

Where we get personal information from:
· Directly from you.
· Family members or carers (in the case of meditation sessions with children, where their parent/carer/guardian provides us with some key health information deemed necessary and appropriate).
· Schools, colleges, universities or other education organisations (in the case where meditation and mindfulness sessions take place within educational settings - this will be done with full transparency and parents/carers/guardians will be part of this process where applicable and the information obtained will be limited to information deemed necessary).
How long we keep information:
E-newsletter: we will store your information for as long as you stay subscribed to the e-newsletter or until such a time that the e-newsletter ceases to exist OR if your email is flagged as being incorrect/inactive. You can unsubscribe at any time.
For queries/complaints: we will store your information for as long as it takes to deal with and close the enquiry/complaint. For queries, your information will be deleted once the enquiry has been dealt with. For complaints, we may need to keep some details on file as a record of how the complaint was brought to a conclusion, but if this is the case we would inform you of this decision and explain why we had grounds that made it necessary to keep your information. We may anonymise some queries/complaints and store them in order to use these as a learning tool.
For those purchasing downloads: we will store your information in relation to financial accounts and will delete this data after the accounts have been processed and submitted for each financial year. Purchase information may be anonymised and stored for forecasting and financial planning after this time.
For those booking places on classes/events/workshops (either paid or free places): we will store your information for as long as you continue to attend classes/sessions with us. We will regularly review which individuals are no longer attending classes and will delete their information. We may contact you to check the information we hold is still correct on a 6 monthly/yearly basis, if you are continuing to attend classes with us.
Who we share information with:
In extenuating circumstances where an individual’s safety or life is at risk, we may share personal information with:
Passing on safeguarding concerns within educational settings.
Emergency services (in the event that an individual requires an emergency service while in on one of our classes).
Organisations we’re legally obliged to share personal information with (if we deem a person in one of our classes to be at life threatening risk to themselves or others).
